Ocean-Strata Integration Streamlines Community Referrals for Surrey Place

We are thrilled to announce a groundbreaking collaboration between OceanMD, Strata Health, Surrey Place, Developmental Services Ontario Toronto Region (DSO-TR), and the Ontario eServices Program to improve the referral process for patients with developmental disabilities. This initiative represents a significant step forward in healthcare coordination and interoperability, promising better, faster care for patients with complex needs.

By integrating Ocean eReferral with Strata Health’s centralized intake and matching technology, primary care providers and hospitals can use the Ocean Healthmap to quickly refer their patients to Surrey Place and DSO-TR. This integration helps connect patient with developmental services more efficiently, eliminating outdated, time-consuming communication methods.

The new pathway offers several key benefits:

  • Efficiency and Ease of Use: Our user-friendly platform integrates directly with Electronic Medical Records (EMRs) to make sending eReferrals easier and more accessible.
  • Visibility and Transparency: Strata Health’s Resource Matching & Referral application enhances coordination among 39 participating organizations, including acute, rehabilitation, and in-home care providers, by eliminating fax-based communication.
  • Reduced Administrative Burden: Automation reduces administrative tasks, allowing healthcare providers to focus more on patient care.

This collaboration isn’t the first time we’ve partnered with Surrey Place! Our teams previously worked together to make their “About My Health” and “My Health Care Visit” tools available in the Ocean Form Library for any primary care provider to use.
