This month we’re highlighting the LookSee Launcher form, a powerful tool for automating the collection of child-development data from birth through 6 years, saving clinics plenty of time vs. managing every milestone individually.
What are the LookSee forms?
The LookSee forms (previously known as the Nipissing District Development Screen (ndds)), are a user-friendly set of child-development checklists typically used by parents, caregivers, and healthcare providers to assess progress from birth to 6 years old. The forms are designed to be simple to use and cover various developmental areas, including: communication, motor skills, social and emotional development, and cognitive abilities.
Why was the LookSee Launcher created?
Pediatric guidelines recommend at least 14 well-child visits between the ages of 0 – 6. Given each of those 14 visits comes with their own comprehensive set of items to assess, this adds up to a LOT of data to collect and manage!
While automating the sending of EMR-integrated Ocean Forms and follow-up reminders is a simple process with Ocean, the sheer number of forms and visit-types required by the LookSee schedule are formidable. That’s one of our innovative clinics decided to build a customized all-in-one form—the LookSee Launcher—to streamline the entire process into a one-form affair.

How do I setup the LookSee Launcher?
- Create a messaging template that includes the LookSee Launcher
- Assign the template to a Patient Reminder for well-child visits
- Once the parents receive the LookSee Launcher, they enter their child’s date of birth
- The launcher checks the patient’s age and shows them the right checklist. If the child is between stages, it will automatically round to the nearest form
- After the checklist is complete, the responses will automatically sync into the patient’s chart!
